Online Diabetes Self-Management Education and Support (DSMES) Class

• All of the same excellent education-just more convenient for you! 
• Over 12 hours of content, with interactive quizzes, and chat access to diabetes educators for seamless support 
• Available anytime, anywhere from the convenience of our Healthie App
• Self-paced learning modules so you can learn your way!
  

In-person Diabetes Self-Management Education and Support (DSMES) Class

• Most comprehensive plan, designed for people with diabetes and pre-diabetes to improve their blood sugar control 
• Accredited by Association of Diabetes Care and Education Specialists (ADCES) 
• Over 8 hours of group education centered around the 5 “Ms” of diabetes management
• Medications
• Meal planning
• Monitoring 
• Movement
• Mindset 
• Trials of fingerstick glucometers and continuous glucose monitors included
• Three individual follow-up consultations with our educators (1 hour each)

Continuous Glucose Monitor (CGM) Data Action Program 

• Designed to help people get the most out of their CGM data
• Whether your goals are to control diabetes, lose weight, or optimize health and performance-this program provides customized recommendations to build confidence and maintain results
